if weren't highlighted in red, that mistake is almost unnoticeable, but still it's a mistake, a mistake that I've already sent to four recipients, who I request to waive my TOEFL score. So ironic...

Hmm... the actual sentence doesn't "sound" that good either. "I'm writing to apply for a TOEFL test waiver" might have been more "natural" English.
Madam is actually not English, so although it looks bad (with "damn" in to) I wouldn't worry about that too much.
Triple check such letters with native speakers. This is one thing you don't want to get wrong :)
